INSTRUCTIONS

 

The team application needs to be completed, and submitted, the rules tell what is generally expected from each team, and the parental consent form must be completed for each participant.  This says the parent has given permission for their child to participate in the program.   

 

The team roster needs to be completed and submitted with the parent consent forms before the season begins. The initial submission of all forms needs to be in by game day.  Amendments may be made until play starts.   The score sheet needs to be completed and submitted to the scorekeeper at the beginning of each game.
